Tax Prep Checklist

Below are some general checklists of items that may be required to prepare your tax return. These lists are not all-inclusive and not all items will apply to everyone. 


All new clients should provide a copy of their previous year's tax return and a copy of their driver's license (and spouse if filing jointly)

Common Items:


  • W-2 forms 
  • 1099-R forms for distributions from retirement accounts
  • 1099-NEC or 1099-MISC for self-employment or rental income
  • 1099-INT, -DIV, -B,  or K-1s for investment or interest income
  • SSA-1099 for Social Security benefits received 
  • 1099-G for unemployment benefits received
  • 1099-SA for distributions from an HSA/FSA
  • Rental property income and expenses
  • 1098-E for student loan interest paid
  • Child care expenses paid (you may have to ask your child care provider for a statement)
  • 1098-T for tuition and education costs
  • Any estimated tax payments made for the tax year
  • Form 1095-A if you purchased your health insurance through the Marketplace 



If your marital status, address, dependents, or banking info has changed from the prior year, please let us know



Less Common Items:


  • If you itemize (most people do NOT), please provide your 1098 for mortgage interest and property taxes, and a record of all of your cash and non-cash charitable contributions.
  • 1099-C for Cancellation of Debt
  • 1099-S for Real Estate sales proceeds 
  • Record of IRA/HSA contributions not made through payroll
  • Receipts for Qualified energy-efficient home improvements
  • Alimony paid/received

  • A record of your business income and expenses. A Profit and Loss statement is most common. 
  • If you keep your records in Quickbooks Desktop, please provide a backup copy of your QB (and the password). 
  • If you use Quickbooks Online, you can send us an accountant's invitation.
  • If you keep your records in Excel, please provide an electronic copy of your excel file.
  • If your business has employees, please provide your year-end payroll reports from your payroll provider.
